TSSAA Physical and Return to Play Forms

A physical must be completed by a physician and on the proper form before a student-athlete will be allowed to try out or participate in a Varsity sport.  These forms, found as "Preparticipation Medical Evaluation Form" on the link from the TSSAA website above, can also be obtained from the Athletic Director or the Head Coach of your sport.
